While I have only dabbled with Pipewire so far and never used RTPmidi, I have PW working fairly reliably in a separate Debian Testing instance.
All I can advise you is that Pipewire in Debian 12 is ancient, but a very recent version exists in the bookworm-backports repository which would be far preferable and may help with your problems.
Set up your sources to include bookworm-backports as detailed here - https://wiki.debian.org/Backports
